Add default example of scale settings (low - high) for KPI questions (such as CES)

In the description the 5-point scale is described, but the labels for the scales are not preset if you add them to your questionnaire.

To make it more clear what the default setting is (also because this is used in the standardized calculations for the KPI's) it would be helpfull to have these scales filled out already.
